Job Summary
Initiate patient care practice as a SHC coordinator, collaborating with health team members to provide required care while acting as a patient teacher and advocate. Conduct home visits to verify the environment is suitable and safe for home dialysis in compliance with CMS regulations. Identify professional role development activities including continuing education, quality assessment, and the clinical application of research findings. Confront ethical challenges ethically with minimal mentor assistance. This role requires one year of registered nurse experience with at least one year in nephrology, dialysis, or peritoneal dialysis. Candidates must hold current, active, unencumbered state RN licensure and CPR certification. The position offers formal and informal professional growth opportunities within a multi-disciplinary team focused on helping patients live better lives through individualized care.
Required Qualifications
- One (1) year of registered nurse experience
- One (1) year of experience as a Nephrology Nurse with dialysis experience required
- in peritoneal dialysis, home hemodialysis, or a combination of both
- Graduation from an accredited nursing school or equivalent
- Current, active, unencumbered state RN licensure
- Current Healthcare Provider certification (CPR) required
- Competent and in-depth knowledge regarding the use of Electronic Health Records
- Ability to organize, prioritize and complete projects independently
- Valid state driver's license and reliable transportation for home visits
