Job Summary

Coordinate dialysis therapy for assigned patients by assessing learning needs, administering medications, and directing safe delivery of home care treatments. Perform ongoing data collection and analysis to modify treatment plans, document outcomes, and ensure accurate laboratory shipping. Train patients and families on equipment operation while identifying home environment barriers and readiness. Initiate emergency responses and collaborate with the interdisciplinary team to update care plans. Participate in staff orientation, quality improvement activities, and CAP advancement requirements. Rotate coverage including nights, weekends, and holidays as needed.

Required Qualifications

  • Graduate of an accredited School of Nursing
  • Current appropriate state licensure
  • Current or successful completion of CPR BLS Certification
  • Must meet the practice requirements in the state in which he or she is employed
  • Must have a minimum of 12 months experience as a RN
  • Successful completion of all FKC education and training requirements for new employees
  • Must be able to lift 50 lbs
  • Must be able to lift chemical and water solutions of up to 30 lbs
  • Must be able to bend over
  • Must be able to stand for prolonged periods
  • Must be able to move machines and equipment of up to 200 lbs
  • Must be able to move patients and assist with ambulation
  • Must be able to work in a health care facility with moderate noise levels
  • Must be able to be exposed to infectious and contagious diseases/materials
  • Must be available for on-call rotation
  • Must be available for night shifts
  • Must be available for weekend shifts
  • Must be available for holiday shifts
  • Must be available for travel to training sites, other facilities or patient homes
  • Must be available to provide essential functions in other locations including patient's home
  • Must be able to perform dialysis treatments in the home
  • Must be able to operate and maintain all Home Dialysis equipment and treatment supplies
  • Must be able to administer medications as prescribed
  • Must be able to document appropriate medical justification and effectiveness
  • Must be able to initiate or assist with emergency response measures
  • Must be able to perform laboratory collection, processing and shipping procedures
  • Must be able to reschedule missed or insufficient laboratory collections
  • Must be able to communicate patient related issues to the physician
  • Must be able to participate in decision-making
  • Must be able to participate in teaching
  • Must be able to participate in leadership functions
  • Must be able to participate in quality improvement activities
  • Must be able to collaborate with the patient
  • Must be able to collaborate with the home care partner
  • Must be able to collaborate with direct and ancillary patient care staff
  • Must be able to collaborate with the Interdisciplinary Team
  • Must be able to collaborate with the Educator, Preceptor or another Registered Nurse
  • Must be able to assess and manage patients' response to home dialysis training and treatment therapy
  • Must be able to follow prescribed predetermined protocols
  • Must be able to perform assessment and identify barriers of the Home Dialysis candidate's home environment
  • Must be able to identify expected outcomes
  • Must be able to update the nursing assessment and plan of care
  • Must be able to ensure patient awareness related to transplant and treatment modality options
  • Must be able to assist in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners
  • Must be able to provide initial and ongoing education to patients and family
  • Must be able to perform all essential functions under the direction of the Supervisor
  • Must be able to perform ongoing, systematic collection and analysis of dialysis data
  • Must be able to make adjustments or modifications to treatment plan
  • Must be able to notify Supervisor or physician as needed
  • Must be able to serve as a resource for health care team
  • Must be able to participate in staff training and orientation of new staff
  • Must be able to ensure correct laboratory collection, processing and shipping procedures are performed
  • Must be able to rotate coverage with other licensed home therapies staff
  • Must be able to work as a Team Leader or designated Nurse in charge
  • Must be able to work with LPNs/LVNs, RNs, Patient Care Technicians and Home Therapy Care Team Assistants
  • Must be able to work in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations

Desired Qualifications

  • Minimum of 1-year experience as a Registered Nurse
  • Home dialysis therapy experience
