Provide housing-based care as a Home Provider in a 2-bedroom condo in Dover, NH for adults with developmental disabilities and individuals who are dually diagnosed. This contractual role offers a stipend and involves sharing a home and life, acting as a caregiver, positive role model, and mentor. Collaborate with a multidisciplinary team and support the individual’s physical and mental wellbeing, decision making, access to community activities, and activities of daily living (ADL) skills. The Home Provider models positive thinking and works to help the individual grow and become independent.
