Job Summary
Support daily living skills including cooking, budgeting, cleaning, and decision-making for an individual with an intellectual disability living full-time in your home. Assist with medication administration, medical appointments, transportation, and community connections while maintaining documentation and communicating with the Mosaic support team. Provide behavioral and physical support as needed to help the individual build confidence, develop independence, and pursue personal goals. Own or rent your current home in Macomb or surrounding Illinois counties, hold a valid driver's license, and be a registered healthcare worker. Initial and ongoing training, dedicated team support, and assistance with licensing are provided.
Required Qualifications
- Must reside in Macomb or the surrounding areas in Illinois
- Must own or rent your current home
- Must be a current resident of the state of Illinois
- Must own or rent your own home, apartment, condo, or townhouse
- Must have active homeowner's or renter's insurance
- Must have reliable transportation, valid driver's license, and current vehicle insurance
- Must have internet access and basic computer skills
- Must be able to complete required screenings, background checks, and training
- Must be a registered healthcare worker (DSP)
- Must be at least 21 years of age
