Home Provider / Shared Living Provider – Colorado

Support daily living skills including cooking, budgeting, cleaning, and decision-making for an individual with an intellectual disability living in your home. Assist with medication administration, medical appointments, and transportation for community participation. Provide behavioral and physical support as needed while maintaining documentation and communicating with the Mosaic support team. Help the individual build confidence, develop independence, and pursue personal goals within a full-time household dynamic. Must reside in Colorado, own or rent your home, and hold active insurance. Initial and ongoing training is provided alongside dedicated team support.
