Home Infusion RN - Independent Contractor (New York)

Conduct in-home infusion therapy visits with patients according to their plan of care, performing comprehensive assessments and initiating preventative or rehabilitative care. Prepare clinical notes, document interventions, and manage patient expectations while serving as a liaison between patients, families, and pharmacy departments. Offer telephone and in-person education on therapy protocols, drug side effects, and adherence. Collaborate with pharmacy and physician teams to develop care plans, establish diagnoses, and regularly evaluate progress. Maintain knowledge of drug interactions and ensure compliance with clinical guidelines under the supervision of the Director of Nursing. This per diem, flexible role supports AmeriPharma's mission of superior clinical outcomes through compassionate home infusion therapy for patients receiving TPN, IVIG, and biologics.
