Job Summary
Confer with physicians and pharmacies to develop initial treatment plans, then administer ongoing infusions and provide hands-on care, management, and evaluation of patient care plans. Complete daily electronic forms in the EMR system, prepare admission, case management, and follow-up visits, and report patient conditions and progress continuously. Implement care plans with patients and families, discharge after physician consultation, and prepare documentation for case conferences, medication changes, and re-certifications. Maintain case conferences, update records, and summarize visit reports while adhering to health and safety guidelines.
Required Qualifications
- Registered Nurse (RN)
- Background check
- Malpractice insurance
- Drug screen
- Associate or Bachelor Degree in Nursing
- Minimum of five years of nursing experience
- Strong Med/Surg, ICU, ER, acute experience
- Current CPR/Basic Life Support certifications (BLS)
- Skilled in accessing Implanted Ports
- Central Lines
- PICC lines
- Strong IV placement skills
- Strong organizational and communication skills
- COVID-19 Vaccinated
Desired Qualifications
- Acute Care setting experience
- Home Health/Infusion experience
- Advanced Cardiac Life Support (ACLS)
- Pediatric Advanced Life Support (PALS)
