Job Summary
Initiate, develop, and implement intermediate nursing plans of care for patients in the home or alternate infusion suites. Perform direct patient care treatments, administer medications, and educate patients and families on prescribed therapy. Assess patient needs through health data access and interviews, then modify treatment plans based on changing status or physician orders. Coordinate discharge planning, prepare summaries, and communicate progress to the clinical team and medical providers. Provide training and mentorship to Infusion Nurse I's, ensuring compliance with agency policies, state regulations, and HIPAA guidelines. Complete real-time clinical documentation and attend required training modules to maintain licensure and competency.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools to communicate and document care provided
- Ability to lift up to 50 pounds
- Ability to exert up to 20 pounds of force occasionally

Desired Qualifications
