Job Summary
Initiate, develop, and implement intermediate nursing plans of care for patients in the home or alternate infusion suite, evaluating progress toward goals. Perform direct patient care treatments, administer medications, and educate patients and families on prescribed therapy and care needs. Modify treatment plans based on changing patient status or physician orders, while assessing physical status through health data access and patient interviews. Coordinate discharge planning, prepare summaries, and communicate effectively with the clinical team, medical providers, and patients to ensure continuity. Provide training and mentorship to Infusion Nurse I's, complete real-time clinical documentation for billing optimization, and respond promptly to patient requests or emergency procedures. Participate in multidisciplinary team conferences, on-call responsibilities, and required training modules to maintain competency.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools to communicate and document care provided
- Ability to lift up to 50 pounds
