Home Infusion Nurse - Part Time - Kona and Hilo

Initiate, develop, and implement intermediate nursing plans of care for patients in the home or alternate infusion suites, evaluating progress and modifying treatments based on changing status or physician orders. Perform direct patient care treatments, administer medications, and educate patients and families to achieve therapy independence. Assess physical status through health data access and interviews, obtaining clarifications on physician orders and providing written summaries of significant condition changes. Coordinate discharge planning, prepare summaries with patient instructions, and communicate effectively with the clinical team and medical providers to ensure continuity. Provide training and mentorship to Infusion Nurse I's, complete real-time clinical documentation for billing optimization, and participate in multidisciplinary team conferences. Respond promptly to patient requests, initiate emergency procedures as necessary, and act as supervisor in the absence of the Nurse Manager.
