Job Summary
Coordinate and provide intermediate direct patient care for acute and chronic conditions in the home or alternate infusion suite under a physician's plan of care. Initiate, develop, and implement nursing treatment plans while administering medications, evaluating patient progress, and modifying protocols based on changing status or physician orders. Perform treatments, educate patients and families on therapy independence, and provide training and mentorship to Infusion Nurse I's as needed. Assess patient needs through health data access and interviews, communicate effectively with the clinical team and providers to ensure continuity, and prepare real-time clinical documentation for billing optimization. Manage PICC/Midline insertion and line care troubleshooting when applicable, and coordinate discharge planning with thorough reporting of care needs. Participate in multidisciplinary team conferences, complete required training modules, and fulfill nursing department on-call responsibilities. Travel up to 100% of the time across Morris and Union County.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools to communicate and document care provided
- Ability to lift up to 50 pounds

Desired Qualifications
