Job Summary
Coordinate patient care, education, and medication administration for home, clinic, hospital, and virtual visits using cardiac or pulmonary clinical expertise. Provide mentorship and clinical training for new nurses through the Central Orientation Program, including competency demonstrations and visit observations. Collaborate with internal and external healthcare teams to address patient condition changes and drug management issues, while partnering with leadership to meet operational and performance improvement goals. Maintain high standards of documentation and practice within regulatory policies and evidence-based best practices. Work 40 hours weekly, including days, evenings, and weekends as needed, with the ability to travel to patients' homes across a large geographic region.
Required Qualifications
- Active RN license in the state where you'll be working and living
- Active, or eligibility to obtain, national certification (CRNI) or other relevant certification
- 5+ years of RN experience
- 3+ years of intensive care nursing experience in Cardiac Care Unit, Pulmonary Care Unit, Open Heart Recovery Room, or in a clinical nursing role focused on the care of patients with diagnoses of Pulmonary Arterial Hypertension or CTEPH
- Strong skills in IV insertion
- Valid driver's license
- Willingness to travel to patients' homes within a large geographic region
- Ability to work 40 hours a week (can include days, evenings, and weekends, per business need)
- Internet connection through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload
