Job Summary
Initiate, develop, and implement intermediate nursing plans of care for patients in the home or alternate infusion suites. Coordinate direct patient care, administer medications, and educate patients and families to ensure safety and compliance under a physician's plan. Assess patient needs through health data access and interviews, modifying treatment plans in response to changing status or physician orders. Provide training and mentorship to Infusion Nurse I's, communicate effectively with the clinical team and providers, and prepare real-time clinical documentation for billing optimization. Coordinate discharge planning, report care progress, and respond promptly to patient requests or initiate emergency procedures as necessary. Participate in multidisciplinary team conferences, complete required training modules, and fulfill on-call responsibilities.
Required Qualifications
- Active and unrestricted RN license in the state of practice
- Minimum of 1 years of nursing experience
- Current CPR certification
- Required licensure to operate a motor vehicle in the state of practice
- Access to a vehicle for business travel
- Proof of liability insurance
- Basic knowledge of computer operating systems and software applications
- Ability to self-evaluate for annual appraisal
- Ability to jointly set professional growth goals with nurse manager
- Ability to apply knowledge in the effective use of nursing technology tools
- Ability to see up close and from a distance
- Ability to see color and peripheral vision
