Job Summary
As a Hospice Nurse Case Manager, you will be responsible for assessing, planning, implementing, coordinating, and evaluating patient care, developing appropriate treatment interventions, and ensuring patient care needs are met. You will supervise other home care staff as necessary. An active RN license in both Vermont and New Hampshire is mandatory, with an ASN required; a BSN or MSN is preferred. This full-time role requires participation in a weekend and on-call rotation.
