Job Summary

Provide direct personal care to hospice patients, including bathing, grooming, dressing, and mobility assistance. Monitor and document patients' physical and emotional conditions, reporting changes promptly to nursing staff. Assist with basic medical tasks like taking vital signs and support medication administration under supervision. Offer compassionate emotional support and companionship to alleviate anxiety for patients and families. Collaborate with the interdisciplinary team to implement individualized care plans and educate family members on basic care techniques. Maintain a safe, clean environment in the home while adhering to infection control protocols. Work four days per week with a rotating weekend schedule and travel within Manitowoc County.

Required Qualifications

  • Current and valid Certified Nursing Assistant (CNA) certification in the state of Wisconsin
  • High school diploma or equivalent
  • Demonstrated experience in providing personal care to patients, preferably in a home health or hospice setting
  • Strong communication skills
  • Ability to work independently in a home environment
  • Valid driver's license
  • Reliable transportation to travel within Manitowoc County
  • Ability to work four days per week with a rotating weekend schedule
  • Attention to detail
  • Documentation skills
