Job Summary
Provide home hemodialysis nursing care and education to HHD patients. Train patients and caregivers on HHD procedures and self-care; manage a caseload of HHD patients including assessments and care planning; monitor treatment progress with home visits and on-call support. Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes. Schedule Monday–Friday, 8-hour shifts with weekends/holidays off; position based in St. Louis, MO. Requires RN license in the state, valid driver's license, and ADN (BSN preferred); involves driving to patient homes and remote/telehealth components as needed.
Required Qualifications
- RN license in the state of practice
- Valid driver’s license
- ADN required; BSN preferred
- Basic computer skills (MS Word, Outlook)
