Home Health West Side Registered Nurse

Conduct comprehensive patient assessments, develop individualized care plans, and perform home visits while monitoring progress and adjusting services as needed. Coordinate interdisciplinary team efforts to ensure efficient care management, educate patients and families on health resources, and serve as the primary contact for all parties involved. Maintain accurate EMR documentation including OASIS assessments within required timeframes and ensure compliance with Medicare and Medicaid regulations. Participate in team meetings, case conferences, and ongoing professional development.
