Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments and creating individualized care plans. Provide interventions for wound care, infusions, and medication management while educating patients and caregivers on daily symptom management. Monitor patient progress, communicate with physicians to adjust plans, and coordinate care across providers to ensure continuity. Complete timely, accurate documentation reflecting the full clinical picture. Requires an active RN license with one year of experience, CPR certification, and a valid driver's license. Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset

- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
