Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers to ensure continuity and complete accurate documentation supporting compliance and outcomes. Requires an active RN license with one year of clinical experience, CPR certification, and a valid driver's license. Required Qualifications
