Job Summary
Provide skilled nursing care in patients' homes on a weekend schedule, working autonomously under an attending physician to develop and execute individualized care plans. Lead care coordination within an interdisciplinary team, perform OASIS assessments as needed, administer prescribed medications, educate patients and families using teach-back methods, and ensure continuity of care through appropriate admissions, transfers, and discharges. Emphasize compassionate communication, attention to detail, and adherence to Medicare/federal guidelines, agency policies, and documentation requirements while prioritizing high-risk patients for visits.
Required Qualifications
- Registered Nurse with current license in the state of employment
- Minimum one-year experience as an RN in an acute care setting
- Home care experience preferred
- Valid driver's license and auto insurance in your name as a driver
- Capable of all physical demands
