Home Health Weekend RN - $7,500 Sign-On Bonus

Job Summary
Manage and coordinate your own home health caseload, completing start-of-care, follow-up, and discharge visits. Perform comprehensive patient assessments, develop individualized plans of care, and ensure OASIS documentation meets compliance standards. Educate patients and families on medications and disease management while monitoring progress and reporting changes to physicians. Collaborate with interdisciplinary team members to deliver one-on-one care in the patient's home. Requires an active, unrestricted RN license with at least one year of experience and willingness to travel within St. Joseph and Elkhart Counties for 12-hour Saturday and Sunday shifts, with one weekday of your choice.
Required Qualifications
- Active, unrestricted RN license
- At least 1 year of nursing experience
- Willingness to travel within coverage area
- Reliable vehicle, valid driver's license, and auto insurance
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
- Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs
- Saturday and Sunday required
- One weekday of your choice
- Full-Time status
- Location: St. Joseph and Elkhart Counties
