Job Summary
Weekend on-call Home Health Physical Therapist in Waterbury, CT. Responsible for on-call visits Friday 4:30 pm–Monday 8:00 am, as-needed and scheduled weekend visits, and admission visits. Develops and adjusts conditioning/rehabilitation plans, conducts assessments, educates clients and families, documents observations and progress, and maintains medical records. May advise on health maintenance and disease prevention, provides case management, and participates in clinical studies. Requires CT PT license and driving. Strong clinical assessment, organizational and communication skills; ability to handle multiple assignments; proficiency with computers/electronic records; Spanish language skills preferred.
Required Qualifications
- Graduation from an approved Physical Therapy curriculum
- Current and valid Connecticut Physical Therapist license (required)
- Driver’s License, Car Insurance, Car Registration
- Recent work experience in home health (preferred)
- Bilingual (Spanish) preferred
