Job Summary
Weekend Home Health Physical Therapist (PT) on-call from Friday 4:30 pm to Monday 8:00 am covering ongoing and scheduled weekend visits in Greenwich, Stratford, New Haven, and surrounding areas. Compensation includes an on-call stipend plus pay-per-visit for visits performed. Responsibilities include developing, implementing, and adjusting a conditioning/rehabilitation plan of care; assessing functional strength; educating patients and families; documenting observations and treatment progress; providing direct patient care as prescribed; reviewing referrals and medical records; tracking prognosis and plans for reevaluation; evaluating treatment outcomes; participating in clinical studies and service education; and performing other duties as assigned. Licensing in Connecticut is required. Driver’s license and ability to travel within the service area are needed. Bilingual (Spanish) preferred. The position emphasizes home health care and patient-focused therapy with documentation in electronic systems.
Required Qualifications
- Graduation from an approved Physical Therapy curriculum
- Current and valid Connecticut Physical Therapist license (required)
- Recent work experience in home health (preferred)
- Driver’s License, Car Insurance, Car Registration
- Strong clinical assessment, organizational and communication skills
- Ability to handle multiple assignments simultaneously
- Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation
- Bilingual (Spanish) preferred
