Home Health Weekend Physical Therapist-Per Diem

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals, while developing and implementing a rehabilitation program consistent with the physician's Plan of Treatment. Provide PTAs and Home Health Aides with clear written care instructions, oversee their performance and documentation, and report patient progress to the physician and care team. Submit all required evaluations, plans, progress notes, and discharge summaries promptly. This role requires a current and unrestricted Physical Therapy license, CPR certification, and a valid state driver's license with personal vehicle liability insurance.
