Home Health Weekend Physical Therapist

Plan and administer prescribed physical therapy treatment for patients onsite in their homes to attain the highest level of physical function. Assess and document patients' physical strengths to support physician evaluations and guide treatment goals. Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient team objectives. Provide clear written care instructions to PTAs and Home Health Aides, overseeing their performance and documentation. Document patient observations, interventions, and evaluations promptly, then report progress to the physician, Clinical Manager, care team, and family while submitting all required evaluations, plans, progress notes, and discharge summaries. Full-time weekend schedule from Friday through Monday.
