Job Summary
Deliver skilled, patient-centered care in a home health setting by performing patient assessments, managing medications, and creating care plans for patients in the Baden, PA area. Educate and empower families on disease management while collaborating with an interdisciplinary team to ensure thorough, compliant EMR documentation. Think critically to solve problems in real time and work independently within your assigned territory. Travel to patient homes, spending approximately 20% of your day driving to and from locations. This weekend role offers a $10,000 first-year success bonus and includes a fleet vehicle program. Required Qualifications
- Active Registered Nurse, RN license in the state you work
- One year of RN experience
- Current driver's license
- Ability to spend ~20% of your day driving to/from patient locations
- Successful completion of a drug screen
- Legally authorized to work in the United States
