Job Summary
Provide in-home skilled nursing care as the primary clinical partner for patients and families in the Twin Cities home health territory. Conduct comprehensive in-home assessments; implement and monitor individualized care plans including wound care, infusions, catheter care, medication management, post-operative and chronic disease management. Educate and empower patients and caregivers, coordinate care across providers, and ensure accurate documentation to support quality outcomes. Schedule involves Friday through Monday daytime visits within the Twin Cities + surrounding suburbs.
Required Qualifications
- Active Registered Nurse (RN) license in state of practice
- At least one (1) year of clinical nursing experience
- CPR certification (or ability to obtain)
- Current driver's license, reliable transportation, and auto insurance
