Home Health Visits Registered Nurse

Conduct head-to-toe assessments, triage urgent clinical situations, and manage acute and post-acute care for clients in assisted living and independent living communities. Perform comprehensive wound assessments, medication reconciliation, patient teaching, and safety monitoring while documenting care at the point of care. Work independently to make real-time clinical decisions, manage a dynamic visit schedule, and prioritize urgent visits across varied home environments. Requires a current Massachusetts nursing license and at least one year of recent hands-on experience in acute, emergency, home health, ICU, or sub-acute settings.
