Home Health Visits Physical Therapist

Make home visits to clients in designated geographic territories throughout Central Anne Arundel County, Maryland. Perform diagnostic tests and measurements, including mobility, range of joints, transfer status, balance testing, and safety assessments. Implement integumentary protective interventions to prevent and promote wound repair. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise the Physical Therapy care plan while participating with nursing in the multidisciplinary team. Educate clients and family members in rehabilitative care to promote independent living. Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current MD Physical Therapist license and graduation from an accredited program. Base pay up to $113,800 annually with comprehensive benefits including medical, dental, vision, 401k match, and paid time off.
