Job Summary
Conduct home health visits to adult and geriatric clients in Waltham, Cambridge, Newton, and surrounding towns while following designated care plans. Administer medications, perform wound care and treatments, and execute doctor's orders with accurate documentation via touch pad tablet. Monitor client conditions, report changes to Clinical or Client Services Managers, and manage case coordination and follow-ups. Requires current Massachusetts LPN license, one year clinical experience, and graduation from an accredited nursing program. Base pay ranges from $45.00 to $53.00 per point.
Required Qualifications
- One year prior clinical experience as a Licensed Practical Nurse
- A current license as an LPN in Massachusetts
- Graduation from an accredited and approved nursing program
