Job Summary
Deliver one-on-one, skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management tailored to patient goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent hospitalizations. Coordinate care across providers, ensure timely documentation, and communicate effectively with physicians to adjust plans. Requires an active RN license with one year of clinical experience, a current driver's license, and CPR certification. Part-time schedule available for those seeking work-life balance.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license, reliable transportation, and auto insurance
- Is CPR certified (or able to obtain certification)
