Home Health Visit Nurse - Evening (1pm-9:30pm CST)

Coordinate and direct care for the home health patient population using advanced assessment, teaching, and decision-making skills. Perform comprehensive physical, functional, and psychosocial assessments while evaluating the home environment for safety and infection control. Develop individualized care plans in consultation with patients, families, and physicians, ensuring appropriate referrals and documentation are completed. Work collaboratively with interdisciplinary teams to facilitate problem-solving and maintain updated EHR records. Evaluate patient responses to interventions and participate in care conferences to address complex needs.
