Job Summary
Assess patients' post-acute needs in collaboration with the multi-disciplinary Halos Home Team and discharge care providers to ensure continuity of care. Conduct on-going patient care and assessments for infants, children, and adolescents in their homes, administering prescribed medications, treatments, and therapies. Coordinate care, educate family members on clinical care to enhance outcomes, and provide preventative initiatives. Serve as a liaison and patient advocate on complicated cases, offering interim care for scheduling gaps and one-on-one training to ensure smooth transitions. Work with varying acuity and scheduling needs, including on-call rotation, while maintaining positive internal and external customer service relationships. Travel approximately one hour from the branch office.
Required Qualifications
- Active PA RN or LPN license
