Home Health Speech Therapist ST Part Time

Deliver quality patient care according to physician orders, including assessments, prescribed treatments, and care planning. Provide written evaluations defining speech therapy needs, goals, and expected outcomes in conjunction with the referring physician's Plan of Treatment. Maintain the plan of care for assigned patients and participate in coordination of services with the health care team, physicians, and family members. Communicate effectively with staff and maintain patient confidentiality while serving as a resource consultant for clinical staff.
