Job Summary
Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Administer diagnostic tests and therapeutic treatments, including audio logic screening. Participate in developing and reviewing plans of treatment and care. Observe, record, and report patient condition changes to supervisors and physicians. Provide instruction on alternative communication systems and counsel patients, families, and healthcare staff. Maintain documentation and submit written reports, evaluations, and progress summaries per policy. Coordinate care activities and assist with discharge planning.
Required Qualifications
- Minimum six months experience as a speech therapist / speech language pathologist
- Current and unrestricted license
- Current CPR certification
- A valid driver's license
- Auto insurance
- Reliable transportation
- Home Health experience
