Home Health Speech Language Pathologist (SLP) / Speech Therapist (ST)

Provide speech pathology or audiology services in accordance with the physician's plan of treatment, accepting only patients referred by the patient care manager. Assess the patient's level of functioning to develop and revise the plan of care, setting appropriate goals and recording treatment reactions for the physician and case manager. Instruct patients, families, and health team personnel in disease prevention phases to promote eventual independence. Recommend hearing tests and check hearing aids when necessary. Maintain biannual competency demonstrations on agency skills. Requires an active Michigan Speech Therapy license, reliable transportation, and valid driver's license. Visit rates range from $63.00 to $105.00 per visit with mileage reimbursement.
