Home Health Speech Language Pathologist (SLP) / Speech Therapist (ST)

Provide speech pathology and audiology services in accordance with the physician's plan of treatment. Assess patient functioning to develop and revise care plans, set goals, and record treatment reactions for the physician and case manager. Instruct patients, families, and health team personnel on disease prevention and independence using agency materials. Recommend hearing tests and check hearing aids as needed. Accept only patients referred by the patient care manager or designee. Maintain biannual competency demonstrations and participate in required in-service programs. Requires an active Michigan Speech Therapy license, reliable transportation, and valid driver's license and auto insurance.
