Home Health Speech Language Pathologist (SLP) / Speech Therapist (ST)

Provide speech pathology or audiology services in accordance with the physician's plan of treatment, accepting only patients referred by the patient care manager. Assess the patient's level of functioning to develop and revise the plan of care using appropriate tests for speech, hearing, and language disorders. Record and report patient reactions and condition changes to the physician and case manager. Instruct patients, families, and health team personnel on disease prevention and independence goals. Recommend hearing tests and check hearing aids when necessary. Maintain competency in required skills on a biannual basis. Requires an active Michigan Speech Therapy license, reliable transportation, and valid driver's license.
