Home Health Speech Language Pathologist (SLP) / Speech Therapist (ST)

Provide speech pathology and audiology services in accordance with the physician's plan of treatment, accepting only patients referred by the patient care manager. Assess patient functioning to develop and revise care plans, setting appropriate goals and instructing patients, families, and the health team on disease prevention. Record treatment reactions and condition changes for the physician and case manager, while recommending hearing tests and checking hearing aids as necessary. Demonstrate competency on a biannual basis and participate in required in-service programs. Requires an active Michigan Speech Therapy license, reliable transportation, and valid driver's license. Visit rates range from $63.00 to $105.00 with mileage reimbursement, contingent on performance.
