Job Summary

Conduct comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs in patient homes. Develop individualized plans of care and measurable treatment goals, providing evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders. Reassess patient progress to modify treatment plans as clinically appropriate while educating patients and caregivers on communication strategies, swallowing precautions, and adaptive tools. Maintain accurate documentation in accordance with CMS and payer requirements, collaborating with physicians, nursing, and the interdisciplinary team to support successful outcomes.

Required Qualifications

  • Degree from an accredited Speech-Language Pathology program
  • Current Michigan Speech-Language Pathologist license in good standing
  • Reliable transportation
  • Valid driver's license
  • Current auto insurance

  • Certificate of Clinical Competence (CCC-SLP)
