Job Summary
Provide patient-centered home health speech-language pathology services focusing on communication and swallowing. Responsibilities include evaluating and treating patients with evidence-based practices, developing and adjusting individualized care plans, educating patients and families, coordinating with nurses/physicians/therapists, completing timely documentation to meet regulatory requirements, participating in care planning and quality improvement initiatives, and adapting treatment based on patient condition. Requires Master’s in SLP, Illinois licensure (or eligibility), CFY completion, at least 1 year of experience (or strong clinical internship), and BLS; authorizes travel within the assigned geographic area and operates in a home-based care setting with autonomy and team support.
Required Qualifications
- Master’s Degree in Speech-Language Pathology
- Active Illinois SLP license (or eligible)
- Completion of Clinical Fellowship Year (CFY)
- Minimum 1 year of experience (or strong clinical internship experience)
- BLS Certification (AHA) – required within 6 months of hire
- Valid driver’s license and reliable transportation
- Travel within assigned geographic area
- Work in patient homes and assisted living settings
