Job Summary
Home Health Speech Language Pathologist - PRN delivers speech-language pathology services in a home-health setting, including comprehensive assessments (OASIS/Agency), treatment planning, caregiver education, and coordination with physicians and care teams. Responsibilities include developing and implementing plans of care in collaboration with the physician, ensuring quality patient care and documentation, supervising Home Health Aide/UP, and maintaining compliance with infection control, regulatory standards, and third-party payor requirements. Requires strong communication, written and verbal skills, ability to assess and treat speech, language, cognitive, and swallowing disorders, and licensure as a Speech Language Pathologist. Bachelor's degree required; Master's preferred; minimum one year of home health experience.
