Home Health Speech Language Pathologist-PRN

Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Administer and interpret diagnostic tests including audio logic screening while assessing disorders of speech, voice, language, hearing, and swallowing. Participate in the development and periodic review of the Plan of Treatment and Plan of Care, observing and recording patient condition changes to report to the supervisor and physician. Provide instruction to patients and families on alternative communication systems and maintain documentation of evaluations, visits, and progress reports. Maintain confidentiality and participate in care coordination and discharge planning. This role is patient-facing and part of Humana's Tuberculosis screening program; candidates must hold a valid state driver's license and auto insurance.
