Home Health Speech Language Pathologist-PRN

Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Participate in developing and reviewing Plans of Treatment and Care. Administer diagnostic tests and therapeutic treatments including audio logic screening. Observe, record, and report patient condition changes to supervisors and physicians. Provide instruction on alternative communication systems and counsel patients, families, and staff. Maintain documentation and submit written reports, evaluations, and care plans. Coordinate care activities and assist with discharge planning while maintaining strict confidentiality. Requires current unrestricted license, CPR certification, and valid driver's license. Pay range $59,300-$80,900 annually.
