Job Summary
Home Health Speech Language Pathologist-PRN responsibilities include evaluating, directing and providing speech/language pathology services in patients' homes or facilities; participating in development and review of Plan of Treatment and Plan of Care; assessing and treating disorders of speech, voice, language, hearing and swallowing; administering and interpreting diagnostic tests; documenting patient progress and coordinating with supervisor/physician; providing instruction on alternative communication systems; counseling patients, families, and healthcare staff; maintaining confidential documentation and contributing to discharge planning and care coordination. Requires minimum six months of home health SLP experience, valid driver license and reliable transportation; compensation noted as pay per visit/unit with Humana driver-safety program participation. CenterWell/Home Health profile and Humana affiliation described. Integrity and compliance with privacy policies emphasized.
