Home Health Speech Language Pathologist - PPV

Provide speech-language pathology services in a home health setting under an interdisciplinary plan of care, including developing therapy plans of care, conducting patient visits for assessment and evaluation, completing OASIS assessments, educating patients, families, and staff, documenting care and updating the EMR, and participating in quality improvement activities. Responsibilities include supervising Home Health Aide as appropriate, coordinating with physicians and care teams, ensuring patient rights and safety, implementing speech and swallowing interventions, and contributing to discharge planning. Requires a Bachelor's degree in Speech Language Pathology (Master's preferred), active SLP license in employment state(s), and at least one year of home health experience; strong communication, documentation, and collaboration skills are essential.
