Job Summary
Conduct speech and language pathology services for clients with speech, language, and swallowing complications under physician direction. Develop plans of care, perform assessments and OASIS evaluations, and provide education to patients and families. Document care accurately in EMR and medical records while participating in interdisciplinary team meetings and quality improvement programs. Supervise home health aides as needed and collaborate with service providers to coordinate patient discharge and outcomes. Ensure compliance with safety protocols, infection control, and ethical standards while maintaining billing documentation.
Required Qualifications
- Bachelor's degree in Speech Language Pathology
- Minimum of one (1) year of experience in a home health setting
- Active and unencumbered Speech Language Pathologist license in state(s) of employment
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Regular standing, walking, and manual dexterity
- Ability to lift and move objects up to 50 pounds
- Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Routine use of standard medical equipment and tools associated with clinical care
