Home Health Speech Language Pathologist-Per Diem

Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Participate in developing and reviewing Plans of Treatment and Care. Administer diagnostic tests and therapeutic treatments, including audio logic screening. Observe, record, and report patient condition changes to supervisors and physicians. Provide instruction to patients on alternative communication systems and counsel families and healthcare staff. Maintain and submit required documentation, including evaluations, visit summaries, and progress reports. Engage in care coordination and discharge planning while upholding strict confidentiality standards. This role offers a pay range of $54.00 - $76.00 per visit/unit, with a full-time equivalent annual salary of $85,400 - $117,500. Valid driver's license, insurance, and reliable transportation are required.
