Job Summary
Evaluate, direct, and provide speech/language pathology services to patients in homes or facilities. Participate in developing and reviewing Plans of Treatment and Care. Administer diagnostic tests including audio logic screening and interpret results. Observe, record, and report patient condition changes and treatment responses to supervisors and physicians. Provide instruction on alternative communication systems and counsel patients, families, and healthcare staff. Maintain and submit required documentation, including evaluations, visit summaries, care plans, and progress reports. Participate in care coordination and discharge planning while maintaining confidentiality.
Required Qualifications
- Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA
- Minimum six months experience as a speech therapist / speech language pathologist
- Current and unrestricted license
- Current CPR certification
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening
