Home Health Speech Language Pathologist

Evaluate, direct, and provide speech/language pathology services to patients in homes and facilities, administering diagnostic tests and applying therapeutic treatments for speech, voice, language, hearing, and swallowing disorders. Develop and review Plans of Treatment and Care, while observing, recording, and reporting patient condition changes and treatment responses to supervisors and physicians. Provide instruction to patients on alternative communication systems and counsel families and healthcare staff. Maintain documentation, submit written reports, and participate in care coordination and discharge planning activities.
